A2P 10DLC Campaign Vetting Delays: Twilio cannot approve 10DLC Campaigns ourselves, and must rely on third parties who control our connections to carriers to sign off. These external processes are creating several week delays for our customers. We continue to escalate these issues and are working to reduce delays wherever possible. Billing for incoming SMS messages

Yes, if Twilio makes a request to your SMS URL then you will be billed. If you are concerned about unexpectedly high message volumes, you can always disable the SMS URL on your phone number (either manually or automatically via the REST API) and Twilio will stop making requests to your application.
